3 sec in the lane

I know it's a violation that rarely gets called, but what defines when a player is in the lane.

Both feet?
What about only 1 foot in the lane?

Am I correct in recalling that a player CAN be in the lane for MORE than 3 seconds if s/he has possession of the ball AND is advancing toward the basket?
