I know high school rules are sometimes different than NCAA and NFL so, is it legal to have 12 men in the huddle? Had this situation MULTIPLE times this week, never called so I want to confirm.

There is no rule in high school  saying you can not have 12 men in the huddle, Now as long as the replaced player leave immediately. What is immediately? A good rule of thumb is if they leave with in 3 seconds

So lining up with 12 then stepping off the field just before the snap is not allowed? Interesting.

Correct.  And all players have to be inside the numbers prior to the snap also, so no lining up with 10 and sneaking one on the field just prior to the snap.
